The cardiology admin team have asked that GPs no longer use printed Clinical Physiology referral forms. A number of these have been received in the last month.

At present all clinical physiology referrals are graded by the cardiologists and so the quickest way to refer is using the BPAC eReferral system. If you feel that only the investigation is required then do state this in the reason for referral, but also include other relevant clinical details.

If anyone else in your practice or peer review groups is using the old clinical physiology referral form please pass on this message to them.
