Feedback from Phoenix Health Centre alerted the DHB to the fact that a tweak to the discharge summary process had led to ACC frequently not being included in the discharge information.

Feedback from Phoenix Health Centre alerted the DHB to the fact that a tweak to the discharge summary process had led to ACC frequently not being included in the discharge information. This issue should now have been rectified but if practices continue to have problems please let me know.
