The GPL team have received a lot of feedback about the new ‘transfer of care’ documents, formerly discharge summaries. This is your chance to point out the problem areas and suggest fixes. The survey form is apparently designed for hospital clinicians as well as GPs, but there is a ‘not applicable to my role’ option to click for the questions that don’t apply to primary care.

“HealthShare, the regional shared service agency that provides MCP, is conducting a feedback survey about the Transfer of Care forms. The goal is to make sure all feedback about the TOC is collected, and to start to prioritise in what order problems should be addressed. After all the results are collated, workshops will be held to finalise specific solutions to identified problems and relative priorities.
The survey takes about 10 minutes to complete and all constructive feedback is welcome.”
