Failure to refer for endoscopy
Health and Disability Commissioner Anthony Hill today released a report finding a general practitioner (GP) in breach of the Code of Health and Disability Services Consumers’ Rights (the Code) in relation to the care provided to a 58 year old man who required an urgent referral for an endoscopy.

Wound management by district nursing service
The Health and Disability Commissioner Anthony Hill has today released a report finding a District Health Board (DHB) in breach of the Code of Health and Disability Services Consumers’ Rights for the care its District Nursing Service provided to a woman with a wound infection.
