The Bay of Plenty Community Care Coordination (CCC) is a demonstration site to test the establishment of a single place for referral and prioritisation for the delivery of community nursing care across the Bay of Plenty.
The Bay of Plenty Community Care Coordination (CCC) is a demonstration site to test the establishment of a single place for referral and prioritisation for the delivery of community nursing care across the Bay of Plenty.
The service went live on 1 March 2018 commencing with referral management and triage for District Nursing Services and the Falls Prevention - In Homes Strength and Balance Programme. Our aim as a service is to ensure that care is directed to the most appropriate community nurse provider and the referral information sent is relevant and complete. With the Falls Prevention - In Homes Strength and Balance Programme eligible referrals will be sent directly to Body in Motion Physio who are contracted to deliver this service.
